I've heard anywhere from 25% once a week to only 25% once every 2 - 3 weeks!
I've heard if you take too much water out of your tank, then you can start your tank cycling again but have never had this problem myself...has anyone?

The frequency comes down to how quickly your nitrates build up. If your tank is heavily stocked and your filters can't keep up then u'll need to increase your frequency. Monitor your levels and find a frequency that suits you. Increasing your filtration is also another option. With my tanks I can get away with a 30% water change ever 3 weeks.
The amount/volume is more dependant on the type of fish your keeping and how hardy they are to parameter changes (PH, temp etc). Tang cichlids are well known for not handling large changes.
Removing the water probably isn't what causes the cycle. Either the new water hasn't been treated correctly and the bacteria is killed off or too much bacteria is removed from the substrate when gravel cleaning. If it has to re-establish itself then u'll see a mini-cycle. Over cleaning your filters can cause this too, make sure u avoid tap water. Personally I'll do one of the tasks above on one maintenance round and then another on the next etc so I don't cause an upset.

We do weekly water changes, but more than 25%. We have done this from day dot and all our fish have grown at a rapid rate and are all healthy, so we are obviously doing something right! Having said that, if we went into our local LFS and told them what we are doing, they would tell us we are
1. Changing the water too frequently and
2. Changing way too much water in one go.
But our fish say otherwise....
