Two Deaths Over Night. What Could It Be?

Please fill out as much as possible below and give us a description of what is wrong with your fish.

Type of fish: deaths-Bolivian Butterflies and 3 weeks ago, two clown loaches.
Symptoms: BB's-fading color. lethargy. Struggling to breathe. no physical markings. CL's- no physical markings. Darting around crazy. Lethargy. Struggling to breathe.
Other tank mates: rainbows. Neons. Black widow tetras. Blue rams. bristle nose. Pleco
Tank size / capacity: 1000L
Type of Food fed: Nutrfin flake
Feeding frequency/amount: 2 pinches once a day
Substrate: inert gravel.
Type of filtration: 3 X FX5's
Frequency of filter cleans: Evey 2 months.
Frequency and % volume of water changes: 50% weekly without fail.
Last water change: 7 days ago. Changes are done automatically on Wednesdays.
PH:6.5-7. 6.8 via PH Probe
KH:15 degrees
GH:>20 degrees. Testing with Sera test kit. Vial remained brownish/green after 20 drops.
Nitrite:0
Nitrate:10 mg/l
Ammonia:0
Phosphate:2mg/l
Water temp:24
Co2: If referring to te Co2 chart then Co2 s >90. But fish are not struggling to breathe and plants pearl slowly.
Medications used recently to date: melafix. One art dose.
Any recent changes..new fish/filters/power outages etc etc: recently developed BGA. Removed most with a total blackout but it is returning after 4 days.
