Type of fish: royal whiptails (all 4 deceased), German Neon Blue Rams (only 1 left), Boesmani Rainbowfish (lost 1,1 still suffering). Asst Guppys & Harlequin Rasboras seem to be unaffected.

Symptoms: lost body condition, started with whiptails and rams, then had a mass die off. Could see little red worms coming out of the anus of the remaining rams and rainbows, after 3 weeks of treatments onky 1 bosemani has protruding worms but i lost another rainbow yesterday, 1 female guppy also seems to be loosing condition

Other tank mates: yoyo loach, khulis, some ottos and about 60 baby guppies

Tank size / capacity: 4x2x2 rimless thickly planted tank.

Type of Food fed: NLS Thera A Mini & NLS Small Fish formular, treats of freeze dried black worm, hikari algae/shrimp wafers.

Feeding frequency/amount: fed once a day, fasting on weekends except for some algae wafers

Substrate: eco-complete black planted & some white sand
Type of filtration: fluval fx-6 filter with standard foam plus 1L matrix and some fine white wool, inline 300w eheim heater and a 36W uv steraliser (switched off for 48hrs following a praziquential dose or duration of a sterazin course).

Frequency of filter cleans: inspected monthly, usually partial clean of wool, rinse the matrix with tank water OR rinse foam (never both in the same month)

Frequency and % volume of water changes: 40% a week or 50% a fortnight depending on work. I did a different schedule during medication treatment, 48hrs after a prazi dose I would gravel vac as much as possible and remove 50% of the water,1 week later redose and follow the water change regime again. When I was dosing Sterazin I did the course for a fortnight and then did a 50% change.

Last water change: last monday, due tomorrow.
PH:7.4-7.8
KH: tapwater is 6-9 (changes each week), i bring it down to around 4-6 with seachem acid buffer (phosphate free).

GH:6-8, changes from week to week, I dont dose calcium or magnesium in my fertilizer regime, so by the end of the week the gh tends to have dropped.

Nitrite: 0 checked with two calibrated test kits
Nitrate: falls to 0 with plant growth, i keep it around 5-20ppm
Ammonia:0 checked with two calibrated test kits
Phosphate: falls to 0 with plant growth, i keep it around 2-5

Water temp: heater is set to 26, this time of year it sits at 27-29

Medications used recently to date:
3 weekly doses of praziquential in addition to a 14 day course of sterazin

Any recent changes..new fish/filters/power outages etc etc:
Added some new plants 6 weeks ago, aside from that nothing new had been in the tank since July when i added some rams

I have tried everything I can think of, I have heard that Kusuri Discus wormer is meant to work wonders but I am struggling to find some without having to wait for it to be shipped, if anyone has any suggestions that would be fantastic.

Come into Aquotix - we have products that will work (same stuff as Kusuri) and other stuff for those resistant to other products.  We use these products in the shop to ensure that the fish we sell are de-wormed before we sell them... so we know what works on what fish :)
