You are going to get the same responses from everyone on here,,,, there should be no ammonia readings at all, as it's severely toxic.
Depending on fish sizes you may be overstocked,,, inadequate filtration or over feeding.
Very small quantities of disease exist in every aquarium,,, even the most sanitary perfectly set up aquariums.
The disease only takes hold of fish when the fish undergo any type of stress that reduces their immune systems.

I've noticed your tank size is small considering the fact the inhabitants you have chosen are not really a good combination for reasons that they get big and don't mix well temperament wise as well as inappropriate long term tank space.
You can get away with mixing fish that shouldn't be mixed in much larger tanks.

There a lot of things that need to be taken into consideration when determining reasons for disease,,, you should see most diseases like this a secondary issue that's caused by a primary issue.
You must determine the primary issue (the cause),,, because there's no point in treating if you don't resolve the primary issue.
