my experience is limited with cichlids but generally its caused by poor water conditions and an impact injury to the eye region...

As Johnno said, the fish gets an eye injury either by fighting or bumping into something. The injured eye then gets infected through poor water quality, if left untreated the fish usually goes blind. However if you do regular water changes and improve the water quality the eye will get better on it's own if you haven't left it to late.

Sometimes a sheath like protector cover, similar to a scab will form while the healing process takes place.
But bad water always contributes to infection in any wound type of damage.
As above mentioned, a good ethics regime keeps the tank sanitary.
Dosing salt such as pool salt from bunnings can contribute to aiding in healing.
Whenever I see a injurie in my fish like this, I just wait it out and be patient and usually comes good.
If damage is to extensive, then it can be irriversable.
